Please complete the following exemption request form.  Please be thorough in your responses.  The COVID-19 Testing Team, including the Testing Coordinator, will review each request on a case-by-base basis and will determine if an exception can be granted based on the information you have provided.   

The following criteria will be considered for exemptions:

-You are under order by a public health official/provider to maintain quarantine/isolation. You will need to provide documentation of this order. 

- You completed vaccination and received your final vaccine dose at least 2 weeks before the testing date.  In addition, you must submit a copy of your vaccination record to the University Health Center.  

- You have proof of a positive molecular (PCR) or rapid antigen COVID-19 test in the last 90 days. 

- You have completed testing at an off-campus location.  You must send documentation of your results for the current week Sunday at midnight - Saturday at midnight.  

- There are extenuating circumstances that must be approved by the COVID-19 Coordinator.

- You are on leave (paid or unpaid) during the week of testing. Being on leave for a partial week may not result in an exemption.   

This Employee Exemption Form must be complete within the week (Sunday – Saturday) of the received invitation.

The COVID-19 Testing Team includes representatives from the offices of the Associate Vice President for Business Services, the University Health Center, Human Resources, and others.  Any personal information submitted through this form will be protected in accordance with applicable federal law (including HIPAA) and state law, as well as JMU policy.  Relevant submitted materials will be added to the employee's record and will only be shared in compliance with law and policy, and only as needed, with JMU offices or public health officials to ensure employee well-being and to respond to the ongoing pandemic.
